I refuse life-prolonging treatment if I am terminally ill with no reasonable chance of recovery. This includes situations where treatment would only extend my life without easing pain or contributing to better health.

If I am seriously ill or in a weakened state due to accident or age without chances of recovery or capability of taking care of myself physically or mentally, I refuse life-prolonging treatment.

I refuse artificial nutrition and hydration (tube feeding, IV fluids) if I am in a terminal condition or permanently incapacitated with no reasonable prospect of recovery.

I refuse antibiotics if treatment would only prolong the dying process without improving quality of life or comfort.

I authorize my wife, Emma Nielsen, as healthcare proxy to have final say in refusing treatment on my behalf if I become unable to make decisions myself. This authority is explicitly granted under Danish law.

I want medical treatment for life-sustaining measures if my physicians believe there is a reasonable chance of recovery to a meaningful quality of life. If my condition is deemed terminal or irreversible with no reasonable chance of recovery, I do not wish to be kept alive by artificial means.

I direct that all measures be taken to keep me comfortable and free from pain, even if such measures may hasten death. Quality of life and dignity are more important to me than longevity under diminished circumstances.

If I am in a persistent vegetative state or permanently unconscious, I do not want tube feeding or IV hydration. If there is reasonable hope of recovery, provide nutrition and hydration as medically appropriate.
